NEW PATIENTS are responsible for 3 very important things before their appointment:
1) You must schedule a quick mandatory consultation with Devin (less than 10 minutes) via Zoom or FaceTime before being seen for your first appointment.
2) You will be EMAILED the consent via docusign during BUSINESS HOURS. If you book during off hours, it will be sent sometime during the next business day. Credit card must be valid to hold appointment but will only be charged if you "no show". Use the SAME email address that you use to book your apt, to complete the docusign consent.
3) You must email a copy of your photo ID to Tammyfacebydevin@gmail.com which will be added to your medical record.
Failure to do any of these 3 things will result in your appointment being cancelled. However, you are welcome to rebook and begin the process again should you miss these instructions or were unable to get them done within 24 hours of booking. - *48 HOUR CANCELLATION POLICY*
- You can cancel online up until 48 hours before your appointment. If you cancel within 48 hours of your appointment time without an excellent reason (especially with no notice at all), you will be responsible for the corresponding cancelation fee noted next to the service you booked.
- *NO MORE DELAYS*
- All of the appointment time blocks have been extended to such extremes that it should now be impossible to experience any delays or wait times. Please plan to be on time!

- Botox - cancellation fee $300
- You do not have to book a lip flip separately! You can get a lip flip during this appointment.
- Lip F L I P & Gummy Smile - cancellation fee $50
- NOT filler (does not add volume into the lips like filler does. A lip flip is just a few units of botox above the top lip, causing it to subtly roll out, creating slightly more height at rest & preventing the top lip from tucking under when smiling. This is one potential cause of a gummy smile, another is caused by the muscles on either side of the nose which we can also treat during this appointment. Plus anything quick (not a brow lift) requiring 20 units or less (bunny lines, nasal tip lift, just in between the eyebrows, just the sides of the eyes, etc.)
- Lip FILLER - cancellation fee $675
- If you have a severe anaphylactic BEE STING ALLERGY, you should NOT get lip filler or dissolver because of cross sensitivity. PLEASE AVOID Advil, Aspirin, Ibuprofen, Motrin, Aleeve & ALCOHOL at least 48 hours but ideally up to 4 days before getting lip filler (they make you bleed badly). Lip injections can trigger a cold sore outbreak so those with a history of cold sores should take Valtrex one day before, the day of & for 3 days afterwards to prevent getting one. Take 1000 mg of Tylenol 1 hour before your appointment & plan to be swollen and/or bruised for about a week, especially the next day.
- Dissolving Lip Filler - cancellation fee $450
- Cannot re-inject same day. Ideally wait 3 weeks.
- Contouring (Cheek, Chin, Jawline Filler) - cancellation fee $450
- Botox & Contouring - cancellation fee $600
- Botox & Lip Filler - cancellation fee $750
